The ICAM Investigation App was designed after years of perfecting the ICAM Investigation Process so that the management and analysis of Data in an investigation is correctly analyzed and interpreted and that the information is presented as a comprehensive Incident Investigation Report.
Incident Investigation Training
The ICAM Investigation App ensures that there are no shortcuts that can be taken when conducting your investigation. The App assists inexperienced investigators by taking the investigator through a step by step investigation process, ensure an investigation is thoroughly conducted.
The ICAM App allow companies to maintain a standardised approach across their organisation and reduce the need for ongoing training of staff as the App trains staff to follow the correct processes when investigating. The App allows the organisation to upload the classification of their internal system processes.
The ICAM APP is currently being redeveloped and will be available again in late 2022. We will update our website closer to relaunch date.
- Data Collection using structured checklists.
- The Classifying of that Data as to whether its Before, During or After the Incident or all three
- It classifies the Organisational Factor Types and allocates to Data Facts
- The App develops a Time Line of the Event
- The App charts the Whys of the Event
- The App completes the ICAM Model by getting investigators to interpret and analyse the organisational factors they have allocated to data facts collected and explain how they are a contributing factor to the Incident
- Task and Environment are completed as part of the ICAM Model Human Error is classified according to the types of
- Human Error and allocates controls for those types of Human Error
- Absent Failed Defences are identified
- Root cause is determined by looking at the Absent Failed Defences and Organisational Factors
- Significant Learnings are completed and corrective actions for Root Cause and other serious non-root contributing factors
- Executive Summary is Finalised
- The App produces a comprehensive report in Word and PDF and allows you to save the file on the IPAD or send to your computer.

Paul Camilleri from Australian Risk Services team was engaged to conduct a comprehensive external Safety Compliance Audit for St John of God Health Care (SJGHC). The ARS team carried out a National Audit Tool (NAT) desktop audit and verification onsite walks across the GroupSafety Management System and 17 SJGHC major sites located in NSW, Victoria and WA. SJGHC found Paul and his team very professional in their auditing approach, dealing with a wide cross-section of clinical and non-clinical staff, in metro, regional and rural settings.

We would like to acknowledge the excellent support and service provided by Australian Risk Services in the provision of independent Plant Hazard Assessments and Safe Operating Procedures (SOP) for 88 items of plant (textile processing machinery) surplus to CSIRO’s needs that were sold recently by public tender. The team at ARS physically assessed each piece of plant for hazards and conducted research to develop a SOP for each piece.
James Cook University (JCU) is reviewing its Workplace Health and Safety management procedures with a view of becoming a sector leader in establishing good WH&S practices. JCU decided to engage Australian Risk Services to conduct Safety Compliance Audits, due to their extensive project management experience in numerous industries and in depth knowledge of safety standards and legislation. The audits included both Townsville and Cairns campuses which have over 400 hundred buildings between them. Paul presented his findings to the board of the University and provided recommendations and corrective action plans. The University has found Paul to be very professional in his auditing approach in dealing with a wide cross section of senior and junior staff.

Paul Camilleri from Australian Risk Services has been consulting to Chep Asia Pacific Container/Crate Division for the past 10 years. During this time he has conducted numerous Safety and Environmental Compliance audits on our operations which have been to the standard of ISO 14001 Environmental Management Systems and AS 4801 Safety Management Systems. He has conducted environmental waste stream assessments and developed CHEP’s Environmental Aspects and Impacts risk register.
